What if instead of Metallia & Beryl, Galaxia had been the one to destroy the Silver Millennium?
What if instead of Black Lady, Wiseman had brainwashed Usagi instead and she had turned evil?
What if the Black Moon Clan had won? What if Wiseman/Death Phantom had never come onto the scene? (Thinking 90's anime/musicals verse here)
What if Kaitou Ace lived?
What if instead of either Endymion or Princess Serenity it was Queen Serenity who Beryl had an unrequited love for?
What if there was a reverse Dark Kingdom arc? Where instead of the Shittenou being the ones to turn evil it was the Inners, and Mamoru and the Shittenou are the stars here (With Usagi being in the position of sort of a "Masked Moon?")

What if during the day the Princess and Endymion met it was actually Venus doing another one of her decoy jobs, and while Endymion thought he was falling for the Moon Princess, it was actually Venus instead?
What if during the Stars arc of the manga Usagi gave into despair and made the decision to destroy the Galaxy Cauldron like Cosmos originally wanted?

Well for those who ask what it would be like if Usagi never became Neo Queen Serenity I suppose Parallel Sailor Moon tells us that.
Also to Tsundereshipper the idea of a series where Mamoru and the Shitennou are the main characters and with a reverse Dark Kingdom arc has been done. It was a fanfic called Cardinal King. In it Mamoru and the Shitennou are the Main characters. Mamoru is the Cardinal King, Jadeite is the Eastern King, Nephrite the Western King, Zoisite the Northern King and Kunzite the Eastern King. Nephrite I believe functions in the Sailor V role in the story. Helios is Mamoru's Guardian Pegasus and his equivalent to Luna. Beryl as a Fairy is the Artemis equivalent of the group. Usagi is Jewel Tiara the Tuxedo Mask of the story The villains are of course the Inner Senshi. Their leader is Diana the Moon Fairy not the cat who is the Moon Kingdom's minister of war and basically was responsible for the death of Princess Serenity in the past in order to stage a war against the Earth and Queen Serenity is the Queen Metalia of the series who was warped by Diana's machinations.
The second season is based more on the R arc with a gender swapped Chibiusa as Mamorin. Sailor Pluto is still alive and heroic. The Inners seem like they turn good in that season and the main villain is Beryl who is Queen of Rose Tokyo and the one who the Doom and Bloom Girls serve. She also functions as the Black Lady of this season being manipulated by Queen Metalia who is the Wiseman of that season and becoming Red Beryl.and even getting a red version of her original outfit on Sailor Moon as her evil version.

Oh yes I've read some of Cardinal King, that's not quite the reverse scenario I was imagining though since Beryl is actually good in that fic. For me, a reverse Dark Kingdom arc would still have Beryl being the Evil Queen corrupted by Metallia, only she corrupts the Inners to her side instead of the Shittenou. (In this version she might be longing for Princess Serenity instead of Endymion too)
Anyways, some more "What if" twists I thought of:
What if Chibi-Usa traveled further back in time than R? Like back during Makaiju arc or even Classic before Usagi & Mamoru awakened to their memories? What if she even travelled all the way back to the Silver Millennium and met her other maternal grandmother?
What if in order to rewrite history the BMC instead traveled back in time to the Silver Millenium themselves to prevent the Princess & Endymion from meeting? Maybe Demande tries to make Princess Serenity fall for him instead in the process? Or what if they traveled back in time to Dark Kingdom era and they tried to team up with the Dark Kingdom in order to prevent Crystal Tokyo from happening?
What if Queen Serenity had reincarnated as well?
What if Pluto broke her taboo and stopped time during Metallia's reign of destruction on the SilMil?

Tbh I always felt that role suited Mamoru more to begin withm as he looked like he was originally going down that route in all versions of the story and is one of the characters most grappling with his sense of identity and the past haunting him what with those dreams. (He even physically transforms against his own will early on in the 90's anime.) Usagi was kind of an out there, odd choice as she never struggled much with identity in the same way.
