This is late, but my guess is that what we see when Usagi confronts Chaos is the Death Phantom skull, like in the manga. Prior spoilers said that we also got the scene where Galaxia met Wiseman, so I assume he'll keep the cloaked guise there, also like in the manga. I think it's unlikely we'll get a drastic redesign for him.
Hey I have actually found a facebook page that shares some pics and clips both official and fanmade from Sailor Moon Cosmos Part 1 and Part 2. This was the image they shared of Sailor Cosmos and Sailor Chaos. Is this how Sailor Chaos is actually shown in Sailor Moon Cosmos Part 2 and is this the official clip of her from said movie?

This is pretty much an interpretation from the manga. In Crystal, Death Phantom and Queen Nehelenia are far different from this. And I don't know why they added Mistress 9 instead of Pharaoh 90, but Mistress 9 is far different from Crystal too.

This is an awesome picture, I've been curious about how this part would be portrayed for a long time. Thanks for sharing it! Even if it turns out it's not from the movie, I still like it a lot.
I think it's more of a redesign. Death Phantom looks like a more evil version of his human self that we saw in flashbacks from before he became Wiseman. Nehelenia looks like a redesign based on the unhinged look she had when she was being defeated. Mistress 9 also looks similar, but with redesigned hair. Metalia is pretty similar, but that's to be expected lol.
If this is from the movie, I like that they included Death Phantom as a human, his backstory as a mass murderer fits his human self being an incarnation of Chaos rather than his persona after fusing with Nemesis. I miss Pharaoh 90 however, I liked having two male and two female incarnations of Chaos.
