Kunzite didn't yell out refresh in the 90's anime because he knew his life with zoisite was a lie. He couldn't bear the thought of their love being a lie. Kunzite was already heartbroken so bringing back his memories would have traumatized him. So he would rather die with the lie.

I think that's a bit of a headcanon.

Imo, the anime is at best vague about how the Shittennou fit into the Silver Millennium. I know, until I watched the CD game I simply presumed they (and probably Beryl) were not human at all and were simply humanoid creatures who shared a common origin with the Youma, Metalia, etc

I think both're pretty likely.

I do agree about the guided growth of rare crystals being used for the Crystal Palace and many other buildings in Crystal Tokyo. I'm going to expand on that in an update...

Ok, so the seal on Queen Metalia started to weaken after all those thousands of years between her being sealed away and the "present" in the 1990s. Queen Metalia was able to reach out to the reincarnation of Queen Beryl (who at the time was an explorer) and the reincarnated Shittennou to go explore the D Point where she was held. There, she awakened the personality of Beryl in that reincarnation and brainwashed the Shittennou once more. The five of them further weakened the seal on Metalia, allowing monsters (Youma) to emerge to drain energy to fully unleash Queen Metalia.
Thankfully, the Sailor Guardians also formed at this time to stop this threat.
